Waiver Forms

All participants must complete the Waiver of Liability, Release, Assumption of Risk & Indemnity Agreement (PDF) prior to shooting. Participants under the age of 18 must have a legal parent/guardian complete the Participant Under 18 Waiver of Liability, Release, Assumption of Risk & Indemnity Agreement (PDF) prior to shooting.

Each form has a button that will allow users to complete and submit the forms online in advance of visiting the range. Participants may also complete the form in-person at the Range office.
